Three major themes Where did patients die? What were the characteristics of the patients who died? What can be done to change the patterns of care?

More died elsewhere missing as some will not have any recent contact with STH hence no recent notes. 200 out of 217 died in hospital (as inpatient) surveyed (92%)

Remembering that the B forms are a smaller sample – STH accounts for exactly 50% of total Oct07 deaths

Alternatives identified subject to acuity of patient (AEP status) plus also the potential for alternatives in terms of GSF and the Liverpool Care Pathway

Great majority were within 1 week of death
