What are the features of a questionnaire? Cover Entry of the questionnaire : explaining the name of distributors, purpose and appreciation to the respondents. Demographic data – gender, age or any details of the respondents. The classification of parts of the questionnaire – the classification can be according to the types of question survey, content or points of the survey. Include clear instruction for every different part of questions.

Insert the page number for a better reference. Use an appropriate font. Provide examples if possible to ensure the respondents can understand better.

Online questionnaire Questionnaire can also be made using the online survey software. It provides an easy gateway to distribute and gain the data as it will automatically evaluate the feedback from the respondents. There are various of online survey software that can be sought easily.
