Assignment 2 Learning Aim D: Individual Treatment Plan Unit 14 Physiological disorders and their care

Assignment 2 criteria P5 Assess care needs of a selected service user with a physiological disorder. P6 Plan treatment to meet the needs of a selected service user with a physiological disorder. P7 Explain how the plan would improve the health and wellbeing of a selected service user. M4 Plan treatment to meet the needs of a selected service user with a physiological disorder, reviewing as appropriate to improve outcomes. D3 Justify the recommendations in the plan in relation to the needs of the service user and advantages and disadvantages of treatment options.

1. Introduction What is the Care Planning Cycle? Explain the purpose and the stages Service User at the centre ASSIGNMENT

2. Background on patient Give their background? What happened in the lead-up to their diagnosis? What are their career and lifestyle goals? How is their general health before the diagnosis? Any family history? ASSIGNMENT

3. Patient's Individual Needs Consider their: Age, gender, ethnicity, religion, culture, marital status, disability etc. How might these factors impact their needs? TREATMENT PLAN
