Warfarin patient stories Aim: To get the patient’s perspective of the anticoagulation services available in NPT Advice sought from the LHB Patient & Public Involvement Group and an advert was placed in the local newspaper Warfarin Patient Stories – Neath Port Talbot LHB

Collecting our story LHB Head of quality & patient safety and LHB pharmacist collected the patient stories GP practice identified 2 patient volunteers (1 patient from each anticoagulation monitoring service) Both patients interviewed at the same time Session recorded for analysis Warfarin Patient Stories – Neath Port Talbot LHB
