Case Conference Slideshow

Meeting Purpose The purpose of this meeting today is my annual case review and transition planning.

Introductions My name is Cindy. This is Mrs. Smith. She is my TOR. This is Ms. Lyons. She is my math resource teacher. This is Mrs. Campbell. She is my gym teacher. This is my mom.

Credits I have 35 credits. I plan to graduate with a Core 40 diploma in May 2020. This means I can go to a… two year college or 4 year college

My Strengths My strengths are….I am very polite, respectful and models great classroom behavior. I display a great work ethic in class. I truly want to please adults, and enjoy helping peers when I can.

My goal statements on my IEP are: My goal for reading comprehension is that I will construct an answer using the RACE strategy 3 out of 4 attempts with 75% accuracy. In Math, I will use theorems to determine whether the geometric figures are congruent or similar 3 out of 4 times with 70% accuracy

Disability The reason I have an IEP is because I have a Specific Learning Disability . This means I have trouble in reading, math, writing and understanding how to do them. Another reason I have an IEP is because I have ADHD. This means it is hard for me to focus.

ACCOMMODATIONS I have the following accommodations: Testing in a small group Use a handheld calculator Extended time on tests A copy of notes Take away an answer choice Extra Textbooks Additional Breaks Test read aloud

Transition Employment: I am interested in being a teacher’s aid. I prefer to work on a team, during the day. To accomplish this I will need my high school diploma. I need support with my interview skills. Living: I am interested in living at home with my family. I prefer to clean and cook. I want to eventually live on my own. To accomplish this I will need a job to pay the bills. I need support paying my bills and budgeting my money. My parents help out a lot. Education and training: I am interested in going to a two year college. I prefer to go to IVY Tech to study Early Education.To accomplish this I will need a high school diploma.

Post-secondary goals Employment: I will obtain a full-time position as a Kindergarten Assistant Teacher Education: I will enroll in a 2 year college of my choice to earn a degree in Early Education

Transition Services Take PSAT,SAT and/or ACT Enroll/participate in School to Work class to practice interview skills, filling out a job application, creating a resume and doing career research

Inclusion setting I have English and Math Service areas Inclusion setting I have English and Math Resource setting I have Biology and School To Work I am happy I receive the support, because I get the help that I need

Concerns of student I have no concerns at this time. I like how I can talk to my Teacher Of Record when I need help calming down I like how I can go to different teachers for math and english help
