Coal Mine Fatal Accident 2012 (No.1) OVERVIEW On Saturday, March 03, 2012, a 32-year old section foreman with 11 years of experience was fatally injured while installing a metal canopy over the operators compartment of a shuttle car. He was positioned in the operators compartment of the car underneath the suspended canopy, when the supports broke and allowed the canopy to fall and strike him resulting in fatal injuries. The hoist used to position the canopy was anchored to a roof bolt bearing plate. This anchor point was not able to support the weight of the canopy and over a period of time broke. There were no secondary cribbing material or means of support placed under the suspended canopy.
