(A) The two most discussed mechanisms for the initiating stimulus are rupture of the vasa vasorum in the adventitia, with spreading haematoma (A1) or a spreading haematoma originating from the vasculature at the base of a penetrating ulcer (A2). (B) The contained crescent-shaped intramural haematoma in the aortic media, with no evidence of an entry tear. (C) The haematoma can progress to aortic dissection, with an identified entry tear into the false lumen (C1) or thrombosis followed by remodelling of the lesion to provide a neomedia (C2).
