HSE LEARNINGS INCIDENT: Root Causes: LEARNINGS: A group of staff left the work site in two private Vehicles an unauthorized journey to Adam. When driver of the vehicle in front noticed heavy dust behind. He then turned to try to find out what had happened. The other vehicle had rolled over and both occupants (IPs) were seriously injured. One of the casualties died on the way to hospital. Root Causes: Driving at high speed No Defensive Driving LEARNINGS: Use PDO approved transportation Get approvals prior to leaving worksite Key Learning: Its safer to use PDO approved transport.
