Day Call for Weekdays

Day Call Flow for Silver (Weekdays) Admit to Late Call Team up to team cap of 11 patients Then admit to Early Call Team up to cap of 11 patients Then admit Remainder of Patients to Gold Medicine

Day Call Flow for Early Call (Weekdays 1200-1500) Early Call Team Take consults for transfer 0800-1500 Admits to themselves until cap of 14 patients Then admit to Late Call Team up to cap of 11 patients Early call team admits and takes consults for transfer from 0800-1500 (see wiki for exceptions) Early call team receives list of patients (from Silver Medicine team) to be seen at 1200 Early call admits to their own team until they hit cap of 14 patients.  Once early call team caps, they begin admitting to the late call team up to census of 11.  Any patients admitted after late call team has census of 11 patients will be admitted to Gold. Then admit Remainder of Patients to Gold Medicine

Day Call Flow for Late Call (Weekdays 1500-1800) Late Call Team Admits to themselves until cap of 14 patients Then admit Remainder of Patients to Gold Medicine Late call team receives list of patients (from Early Call team) to be seen at 1500 (see wiki for exceptions) Late call team begins admitting at 3pm (ends at 6pm) and admit until they hit cap of 14.  All admissions thereafter are admitted to Gold. Late call team hands off patients to be seen at 7pm to Night Float Handoff patients to be admitted to NF at 1900

Day Call for Weekends

Day Call Flow for Early Call (Weekends 0700-1200) Early Call Team Admits to themselves until cap of 14 patients Then admit to Late Call Team up to cap of 11 patients 1. At 0700, the early call team on weekends admits to themselves until cap of 14 à Once the early call team caps, they admit to late call team up to 11 à Once the late call team hits 11, the early call team admits all patients to Gold. 2. At 1200, the late call team admits to themselves to cap of 14 à Once the late call team hits 14, the remainder of patients are admitted to Gold Then admit Remainder of Patients to Gold Medicine

Night Call for Weekdays and Weekends

Night Call Flow (same for weekdays and weekends) Admit no more than 4 patients to Gold or up to 16 Handoff with the NAT occurs the next morning in the Jemez room Then admit to Night Accept Team until 14 cap (but no more than 6) Handoff with Early Call and Backup Teams occurs at 0730 in the Jemez room Oncoming Early Call Team up to 12 Both night float residents start at 1900.  They take hand off of patients to be seen from the late call team in the ER. "Night admits" vs "overflow" At the start of the shift, first print the census for 4 West and 5 West Night float admits patients to the "Night Accept Team" (NAT) listed on amion up to a census of 14 patients, but no more than 6.  If for example, a team is at 8, they would be eligible for 6.  A team at 4 would be eligible for 6 patients.  Then admit no more than 4 patients to Gold or up to 16. -For example, if Gold has 13 patients, admit 3 patients to Gold.   Then admit patients to the oncoming early call team up to 12. Then admit patients to the backup team up to 12. Then all patients admitted thereafter go to Gold.  Gold expands as needed.   The next morning, handoffs will occur with Gold and the Night Accept Team (NAT) at 0700 in the Jemez room. The early call and backup teams will meet with night float at 0730 Oncoming Backup Team up to 12 Gold
