Change Package Your Team Name: Meeting Street Topic Area: Breastfeeding Name of the Intervention: Introducing infant feeding conversations during pregnancy. Primary Driver: Reliable and effective policies and practices supporting families through critical windows for breastfeeding decision- making.

Definition of the Intervention Families will take part in a feeding support visit by a trained CLC at 36 weeks along with the introduction of a feeding curriculum by the family support worker. The family support worker will also complete a Feeding Support Form and a brief survey as part of the 36 week checklist.

Description of the Intervention Around 36 weeks, the family support worker will present families with an introduction to feeding using the prescribed curriculum, Benefits of Breastfeeding, a Feeding Support Form and schedule a visit with a CLC. During the CLC visit, families will be presented with a slideshow, a swag folder and discussion based on the Feeding Support Form. After the CLC visit, the family support worker will complete a brief survey on the interventions provided.

How is the intervention carried out? In order to carry out this intervention a team will need to coordinate with trained CLC’s (who may be members of their team) who are available to go on home visits and meet with families. The family support worker will complete a visit that includes the feeding portion of their curriculum, The Benefits of Breastfeeding sheet, and a Feeding Support Form. After completing this visit, the family support worker will schedule a visit to have a CLC come out to the home to meet the family and provide feeding information and support. The CLC will present a standardized feeding support presentation and tailor any information as neccessary to meet the family’s needs based on the information provided on the Feeding Support Form. Following the support visit, the CLC will provide information as well as any additional support to the family if requested. Lastly, the family support worker will complete a brief survey providing feedback on the interventions.

Evidence that the intervention is effective PDSA Testimony from families, CLC’s and family support workers: - “Being trained as a CLC has been really helpful and I have been able to get to Know my families better. I am able to provide them with the support they are looking for in the comfort of their home.” - “I had struggled with breastfeeding my own son, after being trained as a CLC I feel very confident and determined to support others in their own breastfeeding journey.” - “ Visiting with a CLC was very educational, helpful, and she was very detailed.” - “ Having a CLC come out to meet a family has been very supportive to them, but also informative to me as their worker and I now find myself always checking in with a family to support them with their breastfeeding goals.”

Pre-requisites for the intervention What is needed in order to have success with this intervention? Confidence of staff in basic training Curriculum regarding infant feeding CLC training for some team members or access to CLC’s Benefits of Breastfeeding sheet Conversation about how to use all of the resources CLC basic information package to hand to families

Adaptations (if applicable) We use the Growing Great Kids curriculum but another agency can substitute any feeding curriculum

References/ Inspiration (if applicable) Having the ability to have trained CLC’s on our team and the benefit of them being able to go out and deliver support to families that otherwise would not consider or give up breastfeeding was an inspiration for our team and our CLC’s who have been dedicated to the promotion of breastfeeding. This is our way of introducing families to more of our HFA team and giving our families the support necessary to be successful in their breastfeeding journey.
