What I Do… I provide a specialist Housing Brokerage service tailored to support individuals to access a diverse range of quality, value for money housing options and to make informed decisions about their housing needs. This service is available to people with learning disabilities, autism and mental health needs throughout Trafford. In addition to sourcing and securing housing options, I also: Complete a personalised support plan at the initial visit to identify areas of need Support individuals to negotiate with agents and landlords to achieve the best housing solution to meet their needs. Support individuals to access assistive technology, adaptations or Telecare equipment. Offer free information on the local housing market Signpost to other relevant providers and services in and around Trafford

Choice and control are at the heart of our Housing Brokerage service. We work directly with the individual to ensure that their accommodation meets their independent living needs. Individuals have involvement in all key stages of the process of finding their home, including helping to decide who they live with. We also support the implementation of Personal Budgets for in-house care providers Different approach, with cost effective, settled housing options

The Housing Brokerage Journey I will arrange to meet all individuals within 5 days of referral Initial options plan will be completed within 2 weeks Detailed plan to referrer and individual within 4 weeks Final written summary of options will be provided when case is closed

Referrals Referrals can be made via the CLDT (Community Learning Disability Team) and Greater Manchester West (GMWMH). Clients must have a learning disability, autism or mental health need, and an agreed package of care, to be eligible for the service Out of borough and high cost placements All cases must be completed, or nearing completion, by October 2014

Into the Future If funding for this project is extended, I will work to support individuals in several key areas: Saving deposits Menu of different housing options Rent and Bond schemes Private tenancy agreements Working alongside different Housing Providers Working alongside Housing Strategy
