Selection Criteria for “Champions” Person or Practice that has: High immunization rates > 95 % Spent time with patients Educated staff (front desk to Med Dir.) Conducted reminder/recall on patients Advocated for Immunization/CIRTS Most improved, those with challenging populations that try to catch up Nominated by the Local IAP Coordinator!
