4. Progress on County Plans Organization Actions To Date Suspense Actions Arenac Multi-Purpose Collaborative Body (MPCB) Community Needs Assessment Planning Sub-Committee of the 09/14 – A needs assessment survey was released by the Arenac MPCB to its members. 08/09 – VR attended the subcommittee meeting. They are in the initial stages of the Community Needs Assessment. Kim Bejak volunteered to serve as Chair. 09/17 – Survey responses due to the Arenac MPCB. 09/20 – VR to attend next meeting. The meetings are now being scheduled on a different day than the MPCB meeting.

4. Progress on County Plans Organization Actions To Date Suspense Actions Clare-Gladwin Health Improvement Planning Working Group (Also known as the Health Advisory Subcommittee under the Clare/Gladwin Systems of Care Co-occurring Leadership) 09/14 – Health issues survey distributed to 42 people. 08/26 - Second meeting. There were 10 attendees, excluding the CMDHD facilitator and the CMDH recorders, Kelly Conley and Sandy Merrifield. There were seven “new” attendees. 07/29 – First meeting. There were 12 attendees, excluding the CMDHD facilitator and the CMDH recorder, Sandy Merrifield. 09/20 – Health issues surveys due. 09/23 – Third meeting: setting goals.

4. Progress on County Plans Organization Actions To Date Suspense Actions Health Improvement Planning Working Group Isabella County Community Collaborative (ICCC) 09/10 – First meeting (held after the ICCC meeting). There were 13 attendees, excluding the CMDHD facilitator and the CMDHD recorder, Helen Lee. 10/08 - Second meeting Mecosta and Osceola Human Services Collaborative Body 08/12 –VR presented to approximately 35 attendees, with at least ½ being from Mecosta. 09/14 – Mary Kushion was notified that they intend to form a Mecosta-Osceola health improvement group. First meeting date is to be scheduled.

Progress on County Plans Organization Actions To Date Suspense Actions Roscommon County Health Improvement Planning Working Group Meetings follow the Roscommon Human Services Collaborative Body meetings 09/15 – Second meeting. There were 6 attendees, excluding the CMDHD facilitator and the CMDHD recorder, Kathy Kent. There were two new attendees. They want to do an extensive hard-copy based health issues survey over the next 2 months 08/19 – First meeting. There were 8 attendees, excluding the CMDHD facilitator and the CMDHD recorder, Helen Lee. 10/20 – Third meeting. Survey to be ready for release.
