Job Application Objective: For students to gain experience by practicing filling out a job applications and gathering the information they’ll need.

Info Needed Activity 257 Mother’s Maiden Name. Previous address if you have moved recently. Illnesses or health problems you’ve had. Dates of last physical or vaccination. Availability.

Job Application Every employer has it’s own form, but most ask for the same kinds of information. Dress to impress even when picking up a job application. Neatness counts! Read each question carefully and think about your response before you begin to write. Do not cross out responses. Gives a bad impression.

Job Application Make sure you have a social security number and a copy of your birth certificate. Some jobs require a driver's license as well. Be prepared and know your past supervisor’s name, phone number, and salary. Have references ready as well.
