Diabetes Quality Priority Area of Focus Presented by: Lisa Sims June 15, 2011

Overview Quality priority area and topic Brief overview of EBP Model Application of EBP Model to priority area and topic PICO question Questions and answers

Quality Priority Area and Topic Priority Area – Diabetes Topic – Diabetes Self-Care Management

The Iowa Model of Evidence-Based Practice to Promote Quality Care “The Iowa model of evidence-based practice to promote quality care (Titler, Kleiber, Stellman, et al., 2001) provides guidance for nurses and other clinicians in making decisions about day-to-day practices that affect patient outcomes”(Ciliska et al. 2011, p. 251).

The Iowa Model of Evidence-Based Practice to Promote Quality Care Steps in the Process of EBP Model: (Ciliska et al. 2011) Indentify a problem focused question or knowledge focused question Review and critique relevant literature Identify research evidence that supports the change in clinical practice Implement a change in practice and monitor the outcomes

The Iowa Model of Evidence-Based Practice to Promote Quality Care Application of the Iowa model of evidenced-based practice to improve quality care for research regarding Diabetes Type II and self management will assist in organizing the research in order to implement, monitor and evaluate the outcomes of not only the patients but also the entire team. Through the use of Evidenced-Based Practice, the health care system that is patient-centered and focuses on safety and quality patient care can be successful in improving patient outcomes.

PICO Question In adult patients with Type II Diabetes, how does training and providing education for self management skills compared to no education on self maintenance skills affect the outcomes that are most conductive to improvement?

PICO Format P- Population- The population is adults with Type II Diabetes seen in primary care. I- Intervention- Provide structured group self-management education in Type II Diabetes in an outpatient setting. C- Comparison Intervention- Comparison with no training or education on self-management skills. O- Outcome- Effectiveness of the outcomes that are most conductive to improvement (glycemic control, knowledge, and self care).
