HSE LEARNING INCIDENT: Crew were loading pipe bundles with a Hiab in a pipeyard . Some pipes were out of reach and too close together so the crew used the Hiab to drag a bundle forward. The stack slid forward and crushed the crew leader resulting in a fracture and eventual amputation of his leg. LEARNINGS: Hiab Operators Never lift a load without proper authorization and supervision. Never lift a load unless you are trained and certified Lifting is hazardous operation always stand clear of the dangerous area Never operate the Hiab outside of its weight limit. Ensure the area around a load is clear before lifting Ensure your training is always in date.
