KidneyWise Clinical Toolkit Launched May 2015

About the Ontario Renal Network In 2009, Ontario’s Ministry of Health and Long-Term Care transferred oversight and coordination of kidney care services to the Ontario Renal Network (ORN). ORN, a division of CCO and an agency of the provincial government, manages the delivery of chronic kidney disease services. Delaying the progression of CKD to end stage kidney disease, while improving access to care and services for those living with the disease in Ontario is a main priority for the ORN

KidneyWise Clinical Toolkit The KidneyWise Clinical Toolkit provides guidance to primary care providers on which patients are at high risk of developing chronic kidney disease and recommendations on how to properly diagnose and best manage the patient to reduce the risk for further disease progression. Developed for primary care providers Helps with the identification, detection, and management of CKD Helps PCPs determine which patients are at high risk of developing CKD Provides recommendations on how to properly diagnose and best manage the disease to reduce the risk of further progression Includes the first specialist referral form in Ontario for PCPs

What’s included The KidneyWise Clinical Toolkit includes: An evidence-based Clinical Algorithm that helps with identification, detection, and management of patients with CKD and guidance on which patients may benefit from referral to a nephrologist. An Evidence Summary that offers further clinical detail regarding the algorithm content, including references to clinical guidelines that were used in the development of the toolkit. A Standardized Outpatient Nephrology Referral Form that provides referral guidance by outlining clinical scenarios that would require consultation with a nephrologist along with the appropriate investigations that should accompany the referral. The KidneyWise Clinical Toolkit includes: Clinical algorithm - An evidence-based Clinical Algorithm helps PCPs with identification, detection, and management of CKD and provides guidance on which patients may benefit from referral to nephrology Identify: Identifies patients at higher risk of having CKD and those who should be tested First Nations Inuit Métis (FNIM) patients are at particularly high risk of developing ESRD Detect: Details the types of tests that should be used to detect CKD and based on the test results, guides providers on next steps (i.e. follow-up and referral) Manage: Provides recommendations for management of CKD in primary care to reduce the risk of further progression of the disease
