Bedside Report & Hourly Rounding

Create Expectations for Bedside Report Involve patient and families Greet pt and confirm 2 pt identifiers Introduce self and oncoming nurse Write name on white board Give complete handoff at pt bedside Use 21 point nursing Handoff

Bedside Report Run all lines Confirm IVF hanging and rates by both RNs Check Alaris Pump for correct profile and patient VIN Check line dressings and dates

Bedside Report Look at wounds / complete focus assessment Write nursing Goals / Plan on white board Count Narcotics – document in iView Review orders / task list Sensitive information only outside of room

Hourly Rounding Quality of our rounding is as important as the quantity of rounding. Round on all patients every hour. Document this rounding each hour on the log sheet located at each patients bedside. Exceptions- ICU’s, MUPC. Rounding needs to be multidisciplinary.

The “P’s” of Hourly Rounding Every hour, ask our patients &/or their family the 6 P’s: Pain: Ask patient if they are having pain. Position: Ask pt if they want to reposition (bed to chair, turn, etc.). Personal Care: Ask if pt needs assistance to the restroom. Pumps: Check IV site/fluids hanging. Possessions: Assure call light & possessions close. Path: Assure the pt’s path in room is free of clutter.

Patient Audits Will do a blitz of auditing 10 pts a week for 4 weeks with data sent to Becky Did your last nurse introduce your current nurse? To what extent did you feel involved in the shift report communications Did the nurse allow you to ask questions during the shift report?

Did your nurse listen carefully to your questions? Were your questions explained in a way that you could understand? Do you feel that this type of communication makes you and your family feel included in your care?
