Support Planner (Mental Health Lead) and Support Workers (2) Changing Lives is a stakeholder driven project which is jointly funded to provide intensive support services for 24 people in Scarborough. The project is a collaboration between The Cambridge Centre, Yorkshire Coast Homes, and SWR Mind. The service has recently opened innovative supported accommodation with 6 brand new self-contained flats and 24/7 staff support. The project works in conjunction with all local public and voluntary sector services throughout the community to provide long term solutions to stop cycles of homelessness, substance misuse, mental ill health, offending and other related issues. We now have the following exciting job opportunities: Support Planner (Mental Health Lead) and Support Workers (2) Salary - Support Planner - £23,698, Support Worker - £17,714 37 hpw, Scarborough (posts currently funded until 30.9.16) Applicants must have experience of working with people who have multiple needs and demonstrate an understanding of homelessness and social housing, offending, mental health and substance misuse. A Support Planner takes the lead responsibility for undertaking assessments, preparing support plans with service users to enable them to build their skills and confidence in respect of independent living including budgeting, maintenance and use of leisure time. You will lead on mental health issues and therefore you will require experience in this field. A Support Worker contributes to the assessment process and the development of support plans with service users as above and also supports service users to stay safe. Applicants for both roles must be able to work to flexibly and be able to work early and late shifts, weekends and Bank Holidays on a rota basis. The ability to travel across the area is also required.
