User-Friendly Psychiatric Consultation to Support Integration of Behavioral Healthcare in Primary Care Practice SmartCare PC2 is a Behavioral Health Integration Partnership Program of Vista Hill Funding is provided by San Diego County HHSA Behavioral Health Services

Who Can Contact PC2? All FQHC Clinic Providers/Prescribers – Clinic must sign MOU with Vista Hill. Well need your contact information – Your name – Clinic site – Telephone number – address Allied and Ancillary clinic staff (RN, MA, LCSW, etc.) may call for consults on resources or other management issues.

When you call: Our Nurse Practitioner will receive your call directly. She has experience in behavioral health and in primary care. She will consult with the PC2 psychiatrist and provide a verbal and written FAX report. As needed, our psychiatrist will speak directly with you. A clinic nurse or assistant can initiate a call for your convenience.

How long will it take? We aim to give definitive responses with a 10 minute turnaround, particularly when you have the patient in the exam room. When needed, we may ask for further information or review the relevant literature prior to responding. We will work with you to meet your service delivery needs as promptly as possible. If you have a general question that is not urgent, we will respond as soon as feasible, with no later than one day turnaround.

Who will be responding to my questions? Our nurse practitioner and psychiatric consultant work as a team in responding to your questions. The PC2 clinical staff: Bella Montgomery, CNP Mark Chenven, MD Will Connor, MD Charmi Patel-Rao, MD If it is a general question regarding resources, the nurse practitioner may be the prime responder.

What Types of Call Can I Make? All Ages: Pediatric, Adult, Geriatric Clinical Arenas – General Behavioral Health Conditions Depression, Anxiety, Adjustment Issues, ADHD, Sleep Disorder, etc. – Severe persistent mental illnesses Psychosis, Bipolar Disorder, Borderline Disorders – Substance Abuse – Other Co-Morbid Conditions Medication management support – Side effects & Dosing – Titration Strategies – Contraindications Crisis Assessment and Management Strategies – Suicide Risk, Danger to Others, Risk Management

What we need from you…? Your Name and Clinic site Patients name and demographics Current concerns---- How can we best assist you & your patient? Relevant behavioral health data – Presenting Symptoms and Needs – History of illness – Prior treatment – Medication status and history Relevant Medical concerns: conditions, medications, etc. What is your comfort level in dealing with this patient in question?

Will PC2 prescribe medication? PC2 offers consultative recommendations regarding medication management (med class, dosage, titration, etc.). Prescriptions and ongoing care are the responsibility of the PCP. Follow-up consults are available to support Primary Care Providers with on-going care of complex cases.
