Welcome to the Team Based Cared Kick-off Webinar The webinar will start at 8:00am You are currently muted. To ask a question before the webinar starts, type it out and send to the webinar organizer. Note: questions are sent to the provincial office staff only. Other webinar attendees won’t see them unless the organizer shares both the question and response with all attendees.

Expand the GoToWebinar Control Panel By default the control panel is set to auto-hide. If it disappears, check the top, right-hand corner of your screen for the minimized control panel. Click on the orange arrow to expand to control panel. To turn off auto-hide, click on “View” and uncheck “Auto-Hide the Control Panel”.

Hand Raising The hand raising feature is found on the left-hand side of the GoToWebinar control panel. By default, your hand will not be raised. When your hand is down, the button will look like this: Click on the button to raise your hand if you have a question or a comment. When your hand is raised Click on the button to lower your hand if your question or comment has been addressed.

Team Based Care Targeted Delivery Potential SGLS participants must meet the following criteria in order to access TBC SGLS (with the exception of Foundations of TBC): 1) Be actively working in a team based care environment – defined as at least one practicing physician and at least one other actively practicing health care professional (exclusive of medical office assistants) who are currently working together or who have committed to working together as part of a team delivering patient care. 2) Demonstrate commitment to ongoing practice improvement – defined as any of the following: physician within the practice has completed PSP Practice and Quality Improvement Small Group Learning Series and associated action planning work; a physician within the practice has completed other quality improvement education, such as IHI; a physician within the practice has a history of completing action planning in conjunction with other PSP content; and/or, completion of the GPSC PMH Assessment and associated action planning work.

Targeted Delivery Process This new Delivery Process aims to: Ensure teams who have identified as “change ready” have immediate access to high level support from PSP Enables physicians to take what they have learned and apply it right away in practice Roll out of new targeted process initially focusing on PCN EOI applicants will include: Introductory session ( Foundations of Team Based Care) Subsequent six sessions First two session are IPC and PCC

Learning Objectives - Foundations Identify the value of team-based approaches to care Describe team-based care in a primary care context Summarize evidence to support team-based care Define the competencies that support team-based collaborative care Articulate how the IHI model for quality improvement process can support implementation of team-based models of care
