HEALTH LEADS Started in 1996 with a simple idea: as physicians prescribe medicine and tests, partner with them in their offices to also prescribe housing and better food Founded as “Project Health,” staffs Family Health Desks to write “prescriptions” for social services, housing, food, and other needs Nearly 1,000 college volunteers staffing Help Desks in 20 clinics across the country Since 2010, has served 23,000+ patients 2014: Received a $16M grant from the Robert Wood Johnson Foundation, its largest investor to-date Other partners include Kaiser Permanente, Massachusetts General Hospital, Johns Hopkins Children’s Center, Harlem Hospital Center, Contra Costa Regional Medical Center in Northern California, University Hospitals in Ohio and Dayton Children’s Hospital. Strategy for growth is to: 1) Partner with “lighthouse” health systems in important market segments, 2) Pilot new fee-based advisory services, 3) Pursue organizations with wide systematic reach as partners in disseminating its Health Leads solutions 2016: Launched free Social Needs Screening Toolkit
