Connecting Ontario Health Teams 6

What are Ontario Health Teams? A new model of integrated care, where groups of providers voluntarily work together as one single team. primary care hospital care rehabilitative care home care  community support services residential long-term care health promotion disease prevention  mental health and addictions 7

Ontario Health Teams will Ensure a connected patient experience Provide 24/7 support for patients Offer digital access to records and virtual care 8

Ontario Health Teams will Care for a defined patient population Have a single point of clinical and fiscal accountability Operate under a single budget Follow a defined performance model 9

What does an Ontario Health Team look like? 10

Become an Ontario Health Team All providers are invited to begin the continuous readiness assessment process: Partners assess their own readiness. 1. Self Assessment Based on ministry review, a selection of sites will be invited to submit a full application 2. Full Application Based on ministry review, a selection of sites will undergo an in person visit. 3. In-Person Visit At each stage of readiness, prospective teams will receive supports to move them forward to full implementation 11

Implementation The full implementation of Ontario Health Teams will take several years. Early adopters will help show the positive impact of the teams on quality of care, and patient and provider experience. They will also share important implementation lessons with Ontario Health Teams across the province, through an ongoing intake process. 17
