Who is eligible for services… DDD eligible Transition Student, 21 years of age, and Exiting school this year.

Working Age Adult Policy This policy establishes employment supports as the primary use of employment/day program funds for working age adults. Working age adults are between the ages of 21 and 62. Each individual will be supported to pursue his or her own unique Pathway to Employment

Employment Support Services Individual Employment Group Supported Employment Pathway to Employment (Person to Person) Others (PVS)

Employment Support Services Individual Employment Individual Employment is placement, training and follow- up services necessary to help persons with developmental disabilities obtain and maintain employment in the community. These services may include: Job Development Job Development Job Coaching/Training Job Coaching/Training Job Retention Job Retention

Employment Support Services Group Supported Employment Group Supported Employment is job placement and support services that provide supervised employment in regular business and industry settings for groups of three to eight workers with disabilities.

Employment Support Services Pathway to Employment There are many paths an individual may take towards employment. Paths may vary in length and experiences. The destination is employment!
