Benefits Open Enrollment Effective May 1, 2019

Enroll for the First Time Add/Remove Dependents Passive Open Enrollment = April 9 -23 Enroll for the First Time Make Plan Changes Add/Remove Dependents If you have a life event during the year, you may make changes by notifying HR within 30 days of your event (marriage, birth, adoption, divorce). PLEASE NOTE

Eligibility All regular full-time employees working on average, a minimum of 30 hours per week and has satisfied the waiting period (first of the month following 60 days from the hire date). Who is an eligible dependent? Legal Spouse/Registered Domestic partner Dependent Children up to the age of 26 regardless of school status (Health Care Reform) Note: All changes you make during this open enrollment period will remain in effect through April 30, 2020 unless you have an eligible qualifying event. You have 30 days from the date of a qualifying event to notify HR and make changes to your benefits.

Life and Accidental Death and Dismemberment Insurance Benefits

Employer funded Life and Accidental Death and Dismemberment PharMedQuest provides eligible employees with a Basic Life and Accidental Death and Dismemberment (AD&D) benefit of $25,000. Accidental Death and Dismemberment (AD&D) insurance provides specified benefits for a covered accidental bodily injury that directly causes dismemberment (e.g., the loss of a hand, foot, or eye). In the event that death occurs from a covered accident, both the life and the AD&D benefit would be payable. The Benefits do reduce with Age. 35% reduction at age 65; 50% reduction at age 70 This benefit is at no cost to you, PharMedQuest pays 100% of the cost. Benefit eligible employees are automatically enrolled.   Note: You must name a beneficiary to these benefits. Please remember to update your beneficiary throughout the year if you experience a life event such as marriage or divorce.

Additional Benefits to Life and AD&D Portability / Conversion — With Portability you can keep your coverage if you leave the company or retire, provided you aren’t seriously ill. If seriously ill it may better suit you to convert the policy to a Whole Life policy, which builds cash value. Life Planning Financial & Legal Resources — Specially-trained consultants available to help employees manage emotional challenges of a terminal illness or loss of a loved one. Accelerated Death Benefit — Allows employees to receive up to 75% death benefit in a lump sum, if they are terminally ill. Note: You must name a beneficiary to these benefits. Please remember to update your beneficiary throughout the year if you experience a life event such as marriage or divorce. 

NEW Voluntary Life and Accidental Death and Dismemberment Paid for by you, the employee. Employee Benefit: 5 x your compensation in increments of $10,000 not to exceed $500,0000. Spouse: Up to 100% of employee coverage amount in increments of $5,000 not to exceed $150,000 Child: Up to 100% of employee coverage amount in increments of $5,000 not to exceed $10,000 New Hire Guarantee Issue (no medical underwriting required for new employees) Any elections above the listed guaranteed issue amounts will require the completion of an evidence of insurability form Employee: $150,000 Spouse: $25,000 Child: $10,000

Voluntary Critical Illness – Sun Life Pays You! If you are diagnosed with a covered condition like a heart attack or stroke, critical illness insurance pays a lump sum benefit that can be used however you want. Employee $10,000 Spouse $5,000 Children Wellness $50
