Student Case Management Discussion Session March 9, 2017

Roles and Responsibilities A case manager is assigned to oversee a student's educational/behavioural program. Responsibilities include: Person most responsible for creating and managing the students’ SLP including the coordinating of services Monitors academic progress in consultation with teachers, parents and support staff Discussion with classroom teachers around appropriate adaptations based on the student’s intake, SLP and assessments

Roles and Responsibilities Continued: Coordinate and facilitate connections with school and community support staff including BIT, Counselling, D&A, CYMH, AB Advocate, etc. Set dates for SLP review meetings, invite appropriate personnel (parents, teachers and support staff) and through a collaborative process update SLP goals and action plans including transitioning. Make referrals to SBT when appropriate and represent student at SBT meetings Support student transitioning back to traditional school

Roles and Responsibilities Continued: Consult with support staff to see if a Ministry designation should be pursued.

Alternate Education Requirements An annually reviewed learning plan for each student, either an official Individual Education Plan (IEP) or a Student Learning Plan that clearly defines the objectives for the student, additional services provided as required, progress made, and any transition plans.  An exit strategy to facilitate the student’s transition back into regular school system, continuing education centre, graduation or to work or post- secondary training and education.  Evidence of additional services as required by the student population (i.e. youth workers, drug and alcohol counsellors and/or sessions, etc...)
