LPISD Athletics Committment Multi-Sport Athletes No pass, no play Behavior Accountability
Physicals Date: April 9th Paperwork Permission slips, physical form, and money due by Tuesday April 9th If you are going to your own physician, physicals must be turned in to respective junior high campus by June 7th. Your child is not guaranteed athletics as a class after that date. Not allowed to tryout out for cheer or any sport unless a physical has been turned in and Rank One has been completed.

Clothes/Equipment Boys LPJ: Store will open in May https://lomaxjr2019.itemorder.com Clothes/Equipment Boys LPJ: Store will open in May LXJ: $20 for a set of clothes Will need a pair of shoes for daily workouts and will need a pair of cleats. Will be given one mouth piece. Girls LPJ: Store will open in May LXJ: https://lomaxjr2019.itemorder.com Must wear practice uniform everyday Will need a good pair of every day shoes for athletics and will need shoes for each sport but wait until your daughter makes team
