Additional Forms not Submitted - E-Application A Criminal History Notification Form is required for all applicants. Some forms may be required, depending upon the license type. Here, you select the supporting documents you will be including. Remember, all applications require a criminal history attestation form. Those of you intending to take a class and test at a regional school must select a conditionally approved application and upload your regional school registration form. Applicants requesting a Conditionally Approved application will have the opportunity to upload their confirmation of registration

E-Application not Submitted Properly Time to pay! You will be able to pay within the application. It will take you to “e-pay”. If you are a veteran and have the fee waived you can skip this step by selecting the appropriate box. Keep in mind that after paying you must return to the application. Payment submittal does not submit your application. Get your voucher number from e-pay , you will need it to finish your application. Selecting the military veteran box, exempts them from having to pay for the license at this time.

E-Application not Submitted Properly Check the affidavit and electronic signature. Read and acknowledge the affidavit and sign the application form. Then hit the submit button.

We take approximately 45 days to process applications if there are no deficiencies or concerns with criminal history. We are working on improving/reducing response times even more.

We receive an abundance of paper and electronic information and it is very difficult to match them together if submitted separately. If you did not submit all of the required documentation with your eApplication, this will delay the approval process but you can help by responding promptly to any deficiencies sent to you to prevent further delays in your application. Your eApplication has been received and will be logged into the queue for review. Your application will be processed in 6-8 weeks. Additional Licensing Information Licensing Requirements Check the Status of an Application Please Note: Your application must be approved before you are eligible to register for an exam. If your application is approved you will be sent an approval letter. Once your application has been approved you are eligible to register for an exam.
