ONC Blockchain – Observations 3 year later Debbie Bucci, Office of the National Coordinator for Health IT (ONC)

RECAP Use of Blockchain in Health IT and Health-related Research Challenge 77 response – still referenced today Follow up to formal publications Use of Blockchain in Healthcare Workshop NIST Issue guidance Blockchain in Healthcare Code-A-Thon personal health record identity management API decentralized provider directory Interoperability in Action Webinar Series #4 Blockchain in Healthcare “Oh the Places Data Goes: Health Data Provenance” Challenge 2 winners – one of them blockchain related

Potential Health Care Blockchain Use Case – 3 years later Identity Management (Identity (paitent/provider/researcher), authentication, authorization,) Access Control Provenance Auditing New and Improve Workflows IRB Clinical Trial approval Patient sharing of data Drug dispensing, monitoring Tracking and access to Internet of Healthy Thing Hospital Stays Medicaid Eligibility Off chain record location and viewing Microservices

Federal Government Use Cases Blockchain use cases other agencies are exploring touch many parts and processes of an organization, including: Financial management Procurement IT asset and supply chain management Smart contracts Patents, Trademarks Copyrights, Royalties Government-issued credentials like visas, passports, SSN and birth certificates Federal personnel workforce data Appropriated funds Federal assistance and foreign aid delivery

Observations Health Records are an important part of the ecosystem Many block-chain pilots and proof of concepts occurred over the last 2 years. Waiting for an authoritative source or public entity willing to publish valid claims/information to a node. Vendors have stopped talking about blockchain - now focusing on value add of their service to Health IT environment Early success using Distributed Ledger Technology – private chains within and enterprise to extend existing SOA/SOAP based services – enables agile microservices that support Big DATA - AI type services (HHS Procurement is an example) There are few flavors of blockchain taking most of the market with input support by major companies.   (Etherum and Hyperledger in particular) Slow progress for reward program – earn credit/coins - stood up to support patient participation in a PHR or Research clinical trial
