What is community Health? “The first focus area – ‘community’ – encompasses population groups and the locus (e.g., place, venue, or other unit) of programs, interventions, and other actions.” “The second area – ‘health’ – may be defined differently as a function of a community’s experience and expectations.” “The third area – interventions – encompasses the scope of the intervention(s) being delivered within the community, and reflects the input, needs, perspectives, and goals of communities as they work to improve their health.” “The final area – the ‘science of community health’ – encompasses the methods that are used by the field to develop and evaluate the evidence base that underlies the conception, design, implementation, evaluation, and dissemination of interventions.” Goodman, R. Core principles “Implement and use results of community health assessments (e.g., measuring and characterizing risk factors within, and the health status of, the community) as a core element in improving a community’s health” Engaging community, government, and other partners to create programs working to improve the health of the community Goodman, R. Why is it important? If a community is unhealthy they cannot be: Active Productive Increased health care cost Increased burden on the system and families Increased stress—drinking/smoking And the list goes on…

Whole school, whole community, whole child model (wscc model) (cdc healthy school, 2018) The child in the center is encircled by the five “whole child” tenets. The band around the tenets emphasizes the collaboration needed among the school, health, and community sectors. The ten WSCC components surround the child and provide the full range of learning and health support systems. The community, represented by the outer band, shows that the school remains a reflection of its community and requires community input, resources, and collaboration.

Health people 2020/2030 “Community health workers are frontline public health workers who serve as a bridge between underserved communities and health care systems” (Health people 2020) Webinars are currently taking place with voting on over 30 recommendation for health people 2030 Recommendations include including data and engaging local level to improve health Going beyond state and federal data for reliance on the implementation and institutions of programing

City health dashboard The City Health Dashboard allows you to see where the nation's 500 largest cities stand on 37 key measures of health and factors affecting health across five areas: Health Behaviors, Social and Economic Factors, Physical Environment, Health Outcomes, and Clinical Care. These categories align with those used in the County Health Rankings & Roadmaps, a well-known program that provides health data at the county level. Data come from federal, state, and other datasets with rigorous standards for collection and analysis. The Dashboard team chose these measures, with guidance from a City Advisory Committee, because cities can act on them, they were collected within the last four years, they are updated regularly, and they are backed by evidence. Below, you will find information on each metric including a metric description, data source, years of data, how the measure is calculated, and a link to more information.

Partners in parks In July of 2018 the City of Lake Charlese launched its citywide Partners in Parks initiative aimed at investing private and public dollars to improve park infrastructure and expand programming.

We must shift our thinking… If the top inpatient diagnosis was mood disorders—depression, anxiety, stress, etc. what type of environment are our kids living? How can you improve community health ? THINK OUTSIDE THE BOX—Radio, social media, church The health of the community is a reflection of the health of the people Engage government Engage the faith community Implement new initiatives in the school/community After school exercise programs Go noodle Are you visible to the community, students, and staff How can we improve community health one child at a time? When you work with that child you are working with the parent. Step outside of the box of health education/counseling one on one and reach out in other ways. Social media, the creation of websites, radio, church—is there an active health team in the local church, etc.
