Summing Up This incident could have resulted in a significant injury to the crew member. We need to avoid complacency. Aligning investigations / action items is a positive; - Enables buy in from both client and contractor. - Offshore crew see that we work as a team onshore to keep them safe. It gives a clear and concise message to all parties. This incident reinforces the importance of Stop Work Authority.
