Tardive Dyskinesia (TD) is Associated with Prolonged Exposure to Dopamine Receptor Blocking Agents (DRBAs) TD is defined as involuntary athetoid or choreiform movements (lasting at least a few weeks) generally of the tongue, lower face and jaw, and extremities (but sometimes the pharyngeal, diaphragmatic, or trunk muscles), developing in association with the use of a DRBA for at least a few months1 Timing of DRBA use can help determine if patients have TD1 Neuroleptic withdrawal-emergent dyskinesia occurs when abnormal movement develops shortly after medication use1 Dyskinesia that persists 4-8 weeks after DRBA discontinuation is considered TD1 DRBAs are used to manage:2 Psychiatric disorders Psychosis Depression Bipolar Disorder Gastrointestinal problems Tardive dyskinesia (TD) may result from treatment, for at least a few months, with dopamine receptor blocking agents (DRBAs). Symptoms include involuntary movements affecting the tongue, face and jaw, and extremities Timing of DRBA use can help determine if patients have TD1 Neuroleptic withdrawal-emergent dyskinesia occurs when abnormal movement develops shortly after medication use1 TD can be differentiated from neuroleptic withdrawal-emergent dyskinesia because it persists beyond the 4-8 week window during which withdrawal symptoms are typically seen In addition to antipsychotics, DRBAs are used to treat depression, bipolar disorder, and gastrointestinal problems (such as symptomatic gastroesophogeal reflux or diabetic gastroparesis) American Psychiatric Association: Diagnostic and Statistical Manual of Mental Disorders. Adequate accounting for dropouts (with at least 80% of enrolled subjects completing the study) and crossovers with numbers sufficiently low to have minimal potential for bias. E. The authors explicitly state the clinically meaningful difference to be excluded by defining the threshold for equivalence or noninferiority. 2. The standard treatment used in the study is substantially similar to that used in previous studies establishing efficacy of the standard treatment. (e.g., for a drug, the mode of administration, dose and dosage adjustments are similar to those previously shown to be effective). 3. The inclusion and exclusion criteria for patient selection and the outcomes of patients on the standard treatment are comparable to those of previous studies establishing efficacy of the standard treatment. 4.
