Definition of Transfer : Transfer is the movement of a patient to hospital or from one room to anther, within a unit, or from one unit to anther of the health agency, or from agency to anther or to car / ambulance.

Reasons for transfer: 1-changes in patients condition, when a patients condition becomes serious, such as transferred to CCU if he develops a heart problem or may be transferred to anther agency for services not available in the hospital Example: rehabilitation. 2-patient scheduled for surgery: patient in a medical unit is transferred to a surgical unit to undergo the required surgery.

3Patient requests transfer to a different type of room: from a general ward to a paying private ward. 4-Demand for available beds 5-For isolation of patient 6-For diagnostic procedures 7-Changes in patient's health care needs: from acute to along term’ s rehabilitation agency.

Type of transfers: -Transfer within an agency 2-Transfer outside of the health care agency

Patient transfer : Patients should be accessed for function capabilities before transfer by some examinations: Strength Range of motion Pain Cognitive abilities Movement dysfunctions

Levels of Transfer Independent transfer: patient is able to perform all aspect of transfer without assistance by additional personnel Assisted transfer: patient participate actively with some assistance by personnel Dependent transfer: patient does not participate actively or participate on minimal basis

Levels of Assistance Standby assistance – patient able to perform transfer on their own but may require some verbal cues Close guarding – patient is able to perform transfer on their own but have a likelihood for needing physical assistance such as balance or support Contact guarding – patient is able to perform transfer with minimal assistance; constant contact should be used to insure safety Minimal assistance – patients who can perform at least 75% of the activity Moderate assistance – patient can perform at least 50% of the activity Maximum assistance – patient can perform less than 25% of the activity

Equipment Some equipment used in patient transfers: Wheelchairs Cart or Trolley Hydraulic lift Gait belts Sliding board Sliding Board

Hydraulic Lift Use to transfer patients to chairs or beds Helpful in moving difficult transfer patients( Especially with overweight patients) 450 pounds capacity on average

Gait Belts Gait/transfer belts can wrap around the waist of a patient providing handles for a worker to grasp when assisting or transferring a partially dependent patient Small hand-held slings that go around the patient can aid in transfer by providing handles Handles improves the grasp opportunity for the worker and thereby reduces the risk of accidents

The nurses role in the transfer 1-Give the patient a full explanation of the reasons for transfer ,it will provide the individual with time to adjust, and provide his significant others time to make any change necessary. 2-Inform the patient of date and time the transfer is to occur. 3-Assess whether he is able to walk, or if he requires a wheelchair or trolley 4-Explan about equipment and procedures that may be different in the unit to which the patient is being transferred.

5-Make sure that the family members and friend are notified of the patient 's transfer. 6-Notify the receiving unit or the facility of the date and time of the transfer, his condition and care plan. 7-Notify the admission department of the transfer 8-Arrarnge for transportation if-transferred to another agency eg. Ambulance ,Taxi ,family / friend private car. 9-Gather and assist the patient to pack all his personal possessions. The nurse should check the entire room to ensure that nothing is left behind.

1 0-Recored time, method of transfer and receiving unit in the medical record. 11-Help the patient to transfer to a wheel chair/stretcher.. 12-Report to the nurse in charge and document that the transfer has been accomplished. 13-introduce the patient to the nursing staff in the receiving unit, and remain with him until they have received him into their care.
