I conceived using a donor egg at a licensed fertility clinic Did you use a surrogate? See the surrogacy card Are you married or in a civil partnership? Did your spouse/civil partner consent to the treatment? Has anyone else (yourself excluded) signed the clinic’s parental election form nominating themselves to be the second legal parent? You and your spouse/civil partner are the legal parents. You and that nominated second parent are the legal parents. The second only automatically have parental responsibility if they are named on the birth certificate. Have you signed the clinic’s parental election form nominating someone else as the second legal parent? You are the legal morhter. That nominated person will be the second legal parent of the child, if named on the birth certificate. You are the sole legal parent.
