Case Study: A Member with Uncontrolled Hypertension Engages with the Chronic Care Solutions Program Chronic Care Solutions Program Engagement (Case duration 4 months) Alex calls Health Advocate: His coach suggests that Alex check his BP twice a day and explains how to do it himself. His coach also conducts physician search to help find a cardiologist. Alex attends a Biometric Screening: Health Advocate notifies member of hypertensive BP Oct. 2017 Sept. 2018 Alex, age 56, despite his history of back pain, CAD and an enlarged heart has gone nearly 5 months with no medical or Rx claims. Alex visits his new cardiologist and over the course of 3 visits: Increases BP Rx dose Adds a new BP Rx Check up Three months later, he follows up with his cardiologist: Adherent to Rx Two weeks later Alex ends up in the ER: (BP 200/100) and is started on a new BP Rx
