Where are we now? Applying successes and learning from UK award winning programme for people living with cancer develop a physical activity referral programme for other long-term conditions, including: 93 referrals for arthritis Ongoing exercise after stroke pilot Programme embedded into Aberdeen Falls Strategy. Successful pulmonary rehab classes Developing a referral pathway for people with Parkinson’s Initial development of a referral pathway for diabetes Working with local universities to integrate physical activity into the medical curriculum and to facilitate placements for students within the allied health professions.

Next Steps Expand the programme to meet growing demand across Aberdeen. Moving from a condition specific programme to person centred approach through triage. Work to ensure there is an appropriate clear pathway in place beyond the programme Inc. Range of follow-on opportunities and discounted memberships

More Next Steps The ultimate place to be: Working towards a targeted referral approach to prevention e.g. > People at risk of having a fall People at risk of developing diabetes, COPD etc. Can we make an impact before diagnosis?

Challenges Some of what we will need to see programme growth and development to meet demand: Additional triage staff Additional delivers Additional training - available locally / cost / time commitment Additional volunteers - again require training Development of new and existing partnerships We need to win the trust, hearts, minds and pockets to see the value of investing in physical activity as a preventative measure.
