Patient Questionnaire March 2018 New Street Surgery

Q1: It was easy to make an appointment to see the Doctor

Q3 The Doctor made me feel at ease and gave me the time to explain the reason for my visit

Q5 The Doctor fully understood my concerns and was able to offer advice or treatment

Q6 I wish it had been possible to spend a little more time with the Doctor

Q7 The Doctor discussed a plan of action with me, involving me in the decisions about my care
