Stakeholders for Communities of Care
Topeka Communities of Care Collaboration & Innovation for Community Health The Topeka Communities of Care was established in 2015 to address specific health needs identified in the Community Health Improvement Plan. While the focus of our work is related to Medicare and Medicare consumers, what we do helps all citizens in a community. Our job is to help communities identify the issue(s) and develop strategies to drive change. Following is an overview of the broad focus areas in Kansas Communities of Care: Reducing unnecessary hospital admissions. Reducing hospital readmissions. Reducing the number of Adverse Drug Events (ADEs). Improving community tenure.
