Smart Healthcare Circles of Care

Information is Comforting in a Stressful Time

Family Anxiety has Real Implications During times of illness, family members feel “out of the loop” Urgent, persistent need for any update about loved one’s condition and state of care Often feel helpless to obtain information about their loved one’s status Out-of-town relatives rely on delayed, second-hand information

What is YLOcare? Uses mobile technology to create a virtual waiting room for families and friends of patients Connecting entire families in real time to improve the hospital experience 4

The Solution Open a better line of communication between hospitals and families: The patient designates a manager of his group of individuals to receive updates…The Care Circle, (AKA Circle of Care). 2) Circle members join the network to receive real time updates and can access the Care Circle where they can review messages. Provide updates to the entire Care Circle in real time.

Seamless Integration into Hospital Workflow Web-based application -- No hospital IT required Each care provider simply logs on to send care updates Preapproved messages are chosen from a menu Message transmitted to Circle members through SMS or email The system logs each message for quality control HIPAA and JCAHO compliant
