Integrated Case Management (ICM) Committee CN Subcommittee

What is Integrated Case Management? A strength based, family centered, team approach used to develop coordinated and integrated plans for individuals or families when they have more than one organization involved in their care.

Benefits of an ICM Approach Family centered Builds on strengths Encourages advocacy Recognizes advocacy Recognizes diversity Encourages collaboration and participation Encourages mutual respect Encourages accountability Provides a holistic approach Provides continuity Provides transition planning Encourages least intrusive and intensive intervention.

The Committee Developed out of MCFD adopted policies on integrated service delivery and integrated case management. The sub-committee joined the CN in 2004. The aim of the committee is to make attempts to reach new staff in various agencies such as Education, MCFD, Outreach, Public Health etc. and provide ICM training and support to those agencies.
