Provides help for HIV+ males and females recently released inmates to move forward.

Goal is to provide assistance and reduce obstacles to enroll in THMP (Texas HIV Medication Program), get into medical care, obtain HIV medications and reduce barriers.

Our Services Assist recently released to apply for THMP Increase linkage to care-medical appointments Provide assistance Food vouchers Transportation assistance Pharmaceutical assistance Emergency Pantry Ready eat foods Hygiene Items Referrals Legacy Community Health Clinic on-site HE/RR

Further Assistance Provided With: Housing Referrals Job Referrals Hygiene Products Referrals to clinical case management for long term assistance that may be needed
