Our thoughts on FCP so far: Planning Implementation Outcomes The future

Influencing on the difference of FCP Similar to other roles Very new role- do we have the right training?

Learning from other pilot sites Planned care team/STP GP practice manager GP data team Physio team GPs- mentoring Learning from other pilot sites All came together in about 3 weeks after a planning meeting

Thoughts prior to implementation Clinical: Reasonably confident in managing the various presenting complaints as previous role in primary care Unsure as to how GPs would view us Some concerns over how easy it would be to discuss with GPs (time/availability etc.) Data New notes system (system one) Managerial Ensure staff safe to work in FCP SOP Effect on Out-patient waiting list Backfill and sustainability Experienced ESP to do the pilot

What we are doing Part of the FCP national pilot. Working for 1 practice in Leicester Forest East which has 2 separate premises. 14.5K population size Pilot started on 7/12/18 Two 3 hour sessions a week Initially 30 min appointments- will reduce this to 20 mins once established

Outcome so far as 22/01/19 58 contacts 23 advice and discharge (40%) 19 referred onto physiotherapy (33%) 6 transferred to GP for analgesic prescription (10%) 3 referred to secondary care (5%) 3 referred to podiatry (5%) 3 referred for XRAY (5%) 1 sent for blood test (2%)

Reflections on FCP so far… Diagnosing/managing presenting conditions probably as expected (more foot conditions than expecting!) Patients expectations ?treatment from me? GPs been very supportive & are keen to have us there Really useful being able to discuss analgesia with GPs (analgesia advice - not in my previous role)

Reflection on FCP so far.. Very easy to communicate with GPs “task” message in system one to usual GP/urgent - on call GP Catch GP between patients or discuss over phone with GP on call System one – easy to use (once you know you need to click save!) Few misunderstandings – reception booking patients in - already seen GP & advised physio self referral not FCP

The future… Workforce planning and development Long term funding Sustainability Locality model? Monitor effects on out-patient referral demand
