3 Bridges Stop Collaborative Team How our data base changed lives

3 Bridges Stop Collaborative Team In the beginning we had lots of information on our clients but no clear picture of who they were or what gaps in care there might have been….

3 Bridges Stop Collaborative Team With help from Collaborative staff we created a data base the showed us who our clients were, when & where they were being seen and what they needed to have done.

3 Bridges Stop Collaborative Team Now we could work within our team with other agencies to improve follow up and share care. Creation of an HIV Primary Care flowsheet (IDC/Native Health) Expanded role for clinic RN’s in shared care and case management Regular communication and meetings with STOP Outreach team

3 Bridges Stop Collaborative Team Database and flowsheets require significant initial and ongoing time and energy, but worth it! Enable more standardized, comprehensive care Expanded caregiver roles Expansion of Population of Focus Rapid identification of gaps in care
