What Is It? The Care Portal is a view into an integrated care record for the patient It brings together selected patient data from multiple systems It can notify users when relevant patient events occur It provides secure communication between care providers to support integrated patient care

LPFT Staff Feedback The Portal “enables us to see whether our patients have been admitted [to ULHT] and what outpatients appts they are having. This is helpful when trying to contact our psycho-oncology patients with appointments.” Assistant Psychologist “I’ve found it really helpful in terms of finding out about previous and future outpatient appointments at ULHT which has helped me to ensure that appointments are kept and facilitated.” Physical Healthcare Practitioner

LPFT Staff Feedback “It was useful in finding out whether the patient had been referred to ULHT, whether they were seen and sometimes the outcome of that appointment, ie follow on, discharge etc. …it was useful when we had formulations for the MDT being able to find out whether xrays had been completed and the outcome of these, especially for the Physios.”   Qualified Practitioner, Lincs West Learning Disability Team “I was asked to get a chronology of attendances. We were led to believe the patient had multiple attendances for self injurious behaviour, when it was in fact for a gastro problem. This obviously had a massive impact upon the work we were doing and changed our planned course of care.
