Big/Small Aim (Plan) Reduce readmissions! To determine current ED practices in Ashland County and educate Law Enforcement on process

Change(Do) Created updated ED process for Ashland County after identifying barriers to the current process

Results (Study) 7/1/16-6/30/17 Readmission Rate: 30 Day Readmit Rate 6.8% 59 ED’s 4 30-day readmissions 7/1/17-6/30/18 Readmission Rate: 30 Day Readmit Rate 2.7% 73 ED’s 2 30-day readmissions

Next Steps (Act) Adapt process for ED’s in Ashland County. Meet with local Law Enforcement Agencies (Ashland County Sheriff’s Department, Ashland Police Department, Mellen Police Department & LaPointe Police Department) to train on updated practices Reevaluate readmissions in 3 months

Impact (Lessons Learned) All departments were doing ED’s differently All agencies are now aware of diversion options in the community Express importance of Statement of Emergency Detention by law enforcement officers Ensure consumers are receiving the appropriate level of care Standardized process and training across all departments
