School Based Partnership between Sullivan School District and Missouri Baptist Sullivan Hospital School Nurse Survival Training July 28, 2016

Process from Start Held Community Partnership Workshop – August 2015 MBSH and School met to identify top priorities – September 2015 Share Cost Model Reduce Faculty Absences Improve Student Attendance Improve Student & Faculty Health & Wellness Scheduled visit to an established school partnership to learn how their model was structured – November 2015 Met with School Board for approval – December 2015

Lessons Learned from Established State School Partnerships One size does not fit all Start small Be patient and work through details Use your existing resources Unified partnership from school board, faculty and hospital staff is necessary for success Program does not replace the school nurse Program is optional for students & faculty

Initial Proposed Partnership Structure Full-time Registered Nurse Direct TeleHealth connection between School Campus locations to recruited RN and Clinic Provider Priority schedule for Sullivan School Faculty and Students Direct line from the RN to MBSH Clinic Computer set-up with Next Gen Access Created Standardized Forms FAQ Consent Form Communication Material No Child denied services based on inability to pay

Potential/Expected Results Improve attendance for students who are seen with priority scheduling and are able to return the same day Improve immunization rates as a result of having a local resource for referrals Fewer staff absences Increasing staff participation in health and wellness resource center Strengthen Partnership between MBSH and School

Steps for Implementation Validated school top needs Contract is mutually agreed upon by both MBSH and School Recruited RN with coordination of school for recruitment process Schedule adjusted to accommodate priority scheduling Developed forms and marketing materials Purchased I-pad with secure software for each school campus location Researched Grant opportunity from Missouri Department of Health & Senior Services Formed a subcommittee to meet monthly Established kick-off date Established goals for MBSH and School Identified campus space and fit out requirements

Lessons Learned What Worked Well Introduced Staff to Faculty prior to go-live in February 2016 Started Soft Opening in March 2016 Received 25% Student Consent Forms Developed Refrigerator Magnet Process and Sell Card Developed Signage – Interior/Exterior

Lessons Learned Challenges Some Patients were already established with a Provider Reinforce that program is “Optional” Develop a relationship with RN’s on all Campuses Include MBSH RN recruited to develop relationship with all School Nurses

Next Steps Meeting with another interested district in area Continue to look for improvement opportunities BJC School Outreach Education Programs – exploring funding opportunities Continue to look for a patient testimonial for promotion Training Surface Pro request August 10th Wellness Event August 12th Nurse & Doctor attending Open House in August Scheduling monthly promotions starting in August Developing educational materials specific for young women Educational sessions offered by MBSH Providers to Faculty
