Community Health Needs Assessment WHAT IS IT? Purpose: Identify and describe factors that affect the health of a community, and identify available resources to address those factors. Process: Form an advisory committee Collect and review data Select 3 priority health issues Identify future partners/resources Develop a written implementation strategy Timeline: Began September 2018 Ending May 2019 Top 10 Priorities identified: 1. Mental Health 2. Stress 3. Depression 4. Chronic Disease 5. Suicide 6. Obesity 7. Transportation 8. Substance use 9. Access to healthy and affordable food 10. Access and awareness to services and resources & dental care (tied) Next steps Moving forward, the steering committee team will develop a written Implementation Strategy (CHIP)to address the three priority health needs identified during the assessment process. This plan will be created in partnership with the Fairview Northland Community Health Steering Committee, public health, and other community members, to be adopted by the Fairview Northland Board of Directors by May 15, 2019, and executed during years 2019-2021.

Community Health Needs Assessment WHAT IS IT? Top 3 Priority Areas identified: Mental Health Healthy Lifestyles Access to care and resources These priority areas were left intentionally broad to allow for variation during the planning process.

Community Health Needs Assessment DISPARITIES At or Below Poverty Level: All Residents: 12% American Indian: 33% Two or More Races: 17% White: 10% Socioeconomic factors: Fairview contracted with Wilder Research to research the social determinants of health for Mille Lacs and Sherburne Counties. The determinants reviewed were – socioeconomic status, education, employment, housing, and transportation. Health disparities adversely affect groups of people who have systematically experienced greater obstacles to health based on their racial or ethnic group; religion; socioeconomic status; gender; age; mental health; cognitive, sensory, or physical disability; sexual orientation or gender identity; geographic location; or other characteristics historically linked to discrimination or exclusion. Each of the leading causes of death is related to unique health disparities, but also shares several general similarities. For example, health disparities adversely affect people of color and indigenous people for cancer, heart disease, unintentional injury and chronic lower respiratory disease. Health disparities exist between those with the highest income levels and the lowest, as well as between the insured and uninsured. Those in the lowest income level without insurance have the greatest health needs and are most challenged in gaining access to high-quality affordable healthcare. Mille Lacs County: Median household income in Mille Lacs County is lower than the state average,10 but disparities exist by race, gender, and age. The current median household income in Mille Lacs County is $51,232, which is similar to 2000 levels. White residents have the highest median income at $52,025 per year. Current median income for persons of color is nearly $13,000 less than that of white residents. Single-female and single-male headed households saw their incomes decline approximately 13 percent between 2000 and 2016. Mille Lacs county residents under age 25 and 65 and older have much lower incomes ($38,819 and $31,770), while those 25-44 and 45-64 years of age have incomes well above the county average ($61,042 and $56,830).

Social Determinant of Health HOUSING Housing affordability: Housing affordability impacts an individual’s or family’s economic stability. When a household is cost-burdened — paying more than 30 percent of their income on housing — there is less income to pay for basic needs, including health care costs. The number of cost-burdened households in Mille Lacs and Sherburne Counties rose between 2000 and 2016, with renters being more likely to be cost-burdened than home owners. Mille Lacs county residents saw a bigger increase in owner cost-burden households, than Sherburne (seven versus two percent), however most of the overall cost-burden increase was due to the 12 (Mille Lacs) and 18 (Sherburne) percent increases for renters.

Social Determinant of Health TRANSPORTATION Employees in Mille Lacs County, MN have a longer commute time (27.9 minutes) than the normal US worker (25 minutes). Additionally, 4.44% of the workforce in Mille Lacs County, MN have "super commutes" in excess of 90 minutes Additionally we have a slightly higher percentage of households without a vehicle and whom also use alternative transportation to get to work (in comparison to our neighboring county). Access to reliable transportation, regardless of the mode, helps ensure residents can travel to work, purchase healthy foods, access health care services and other supports, and socialize with others, which all are necessary for health and a high quality of life. Few households in Mille Lacs and Sherburne Counties were without a vehicle, but Mille Lacs County saw a decrease of two percent in the use of alternate form of transportation to get to work, Sherburne County, saw a two percent increase. Social Determinant of Health EDUCATION, ECONOMIC & EMPLOYMENT Uninsured Rate Mille Lacs: 4.1 MN: 6.3 Unemployment Rate: Mille Lacs: 3.7 MN: 3.8 2018 US Census Bureau Education Addressing disparities in educational attainment is important because individuals who earn a bachelor’s degree or higher are more likely to secure full-time employment and higher earnings. A college education is a pathway to acquiring income, benefits, and assets, all of which are strongly associated with better health. Education rates are going up for all groups in Mille Lacs and Sherburne Counties, however these counties still have fewer people who have received their bachelor’s degree or higher compared to the state of Minnesota overall (35 percent).13 Sherburne County residents are more likely than Mille Lacs County residents to have obtained a bachelor’s degree or higher (27 percent versus 16 percent). Education rates of white residents reflect the overall county rates and very little can be said about education rates for other residents in the county due to small population sizes and unreliable data.

Social Determinant of Health FOOD ACCESS For those living in low-income communities and communities of color, healthy food is simply out of reach. Finding quality fresh food means either traveling significant distances or paying unaffordable prices for wilting vegetables and overripe fruit. With these burdens, it is no surprise that these same communities face the highest risks of obesity, diabetes, and other preventable food-related health challenges.

Social Determinant of Health OTHER TRENDS Drug overdose deaths continue to increase in Minnesota. In 2016, death certificates indicated that 675 deaths were a result of drug overdose, compared to 538 in 2015. Drug overdose deaths include accidental poisoning by drugs, intentional self-poisoning by drugs, assault by drug poisoning, or drug poisoning of undetermined intent. Statewide opioid-involved deaths increased 18 percent from 2015 to 2016. Deaths from overdose involving methadone and prescribed opioids, such as codeine, oxycodone, or hydrocodone remained stable; however, there was an increase in deaths involving heroin and other synthetic opioids.

Community Health Improvement Plan WHAT IS IT? Purpose: A vision for the community’s health that addresses long-term, systematic efforts to address public health problems within the community. This is the community’s plan not the community health boards plan for the community. Process: Prioritized issues with advisory committee Narrowed down to top 3 priority areas with advisory committee Conducted a root cause analysis of top 3 areas Currently developing a structure where the root causes are the final priorities. Next Steps: Finalize structure, formulate goals, strategies, and define roles with partners Implement, monitor, and revise with partners Timeline: Began February 2019 End date March 2020
