Fig 1 (A) and (B) A primary polytetrafluoroethylene (P TFE) loop set, made of a single CV-5 suture and a small PTFE pledget, was placed on the papillary muscle with another pledget. The length of the each loop is much shorter than that between the tip of the leaflet and target papillary muscle. (C) The efficient length of the neochordae was adjusted with the secondary loop. (D) Fixing a knot using a reusable neurosurgical clip. (E) Multiple creations of neochordae are feasible with loop-in-loop technique.
