Fig 1 A, The venous conduit, consisting of the Giacomini and the small saphenous (SSV) veins harvested in continuity, has been mobilized (case 1) in its entirety and is shown lying upon the anteromedial aspect of the lower limb, with the patient in the supine position. The tip of the forceps, depicted at the far right, rests on the upper thigh, and the ligated SSV top-end is just anterior to the knee. The Giacomini vein is the graft segment on the right of the ligated SSV top-end. B, Shown are the Giacomini vein, thigh extension of the SSV (right), and proximal SSV (left) harvested in continuity just before the popliteal aneurysm exposure; the saphenopopliteal junction is depicted at the back of the knee. A single surgical incision was used, with the patient prone (case 2). The venous conduit's diameter marked preoperatively on the lateral aspect of the leg ranged from 4.3 to 4.5 mm.
