1 10 5 30 150 600 1 catastrophic incident 10 major property damage incidents 5 major injuries or illnesses 10 5 30 minor property damage incidents 30 150 150 minor injury/illness incidents Catastrophic is an amputation, total incapacitation or death of an employee or extreme property damage or total loss of equipment Major is an injury or illness medical treatment beyond 1st aid and/or loss of time with possible long term effect or heavy property damage / extended loss of equipment Minor is an injury/illness requiring only minimal treatment, no lost time or restricted activities or minimal property/equipment damage with no loss of use. 600 For every 600 “Near Hit”incidents with no visible injury or damage (normally un-reported) * Estimates based upon published data from UK, Canada and USA safety organizations
